Forms for parents

To Apply to St. Ambrose Preschool:

Diocesan application form : to apply, please fill out this Diocesan Application form. Mrs. Konczal ([email protected]) is happy to answer any questions you have regarding the application.
Submit a $75 non-refundable application fee. (checks written to Saint Ambrose School)
If your child is attending another preschool program or childcare center please give a copy of this Teacher Comment Form to his or her teacher or caregiver. Once complete please send to: Saint Ambrose School, 3827 Woodburn Road, Annandale, VA 22003. Attn: Marybeth Cuccias

Once Accepted:
Once you have received your acceptance letter, please submit a $225 non-refundable registration fee within two weeks. The forms below should be filled out and submitted between June 1 and June 30. An original birth certificate will need to be presented prior to the first day of school.

Virginia School Entrance Health Form: this form is required for all students.
Parental authorization & Permission for Emergency Care form : Indicates who is allowed to pick up your child from school.
Media Release form: this form allows you to indicate whether or not you will allow the school to use your child’s image, voice, or name for publications, videotapes, audio recordings, photographs, etc.
Child's family information form: this form allows you to tell us about your child and family so that the teachers will be aware of your child’s unique needs and circumstances
